Clinical Case Manager I/II


Under the direction of the Permanent Supportive Housing Manager, the Clinical Case Manager provides Intensive Case Management Services (ICMS), working directly with program participants in a PSH site to develop, arrange and coordinate treatment services. The CCM provides support with housing maintenance, eviction prevention, and connection to medical, mental health and/or other various community resources that help improve a client’s quality of life. The targeted population for this project-based site are Formerly Homeless Veterans (Other Than Honorable).


Client Engagement Specialist


The Client Engagement Specialist provides face-to-face services to clients who are involved with state, county, and city entities (e.g., probation, courts), engages with clients, and determine which Substance Use Disorder services are most appropriate for individuals based on an ASAM brief triage screening.

Requirements: must be a registered or certified counselor with RADT/CADC/CATC or have a Masters in Social Work or Marriage & Family Therapy.

Substance Use Specialist I

The Substance Use Specialist I is responsible for conducting screenings/assessments, educating clients/patients about their different treatment options and facilitating individual counseling and group sessions, provides case management services, develops treatment plans, coordinates care and works with inter-disciplinary team to provide continuity of care.
