Clinical Case Manager 

 

Under the direction of their Team Lead, the Clinical Case Manager (CCM) position will provide Intensive Case Management Services (ICMS) on-site at a permanent supportive housing site to clients experiencing chronic medical conditions, mental health, and/or substance use. The CCM will provide hands-on support with housing maintenance, eviction prevention, and connection to medical, mental health, and other various community resources that help improve a client’s quality of life. The CCM role will ensure services embrace HHCLA's mission and core values throughout all aspects of client services and support provided.

Clinical Case Manager

 

Under direction of their Team Lead, the Clinical Case Manager (CCM) position will provide scattered-site Intensive Case Management Services (ICMS) to clients experiencing chronic homelessness, chronic medical conditions, mental health, and/or substance use throughout Los Ángeles County. The CCM will provide on-hands support with navigating obtaining housing, housing maintenance, eviction prevention, and connection to medical, mental health and other various community resources that help improve a client’s quality of life. The CCM role will ensure services embrace HHCLA's mission and core values throughout all aspects of client services and support provided.

 

Client Engagement Specialist

 

The Client Engagement Specialist provides face-to-face services to clients who are involved with state, county, and city entities (e.g., probation, courts), engages with clients, and determine which Substance Use Disorder services are most appropriate for individuals based on an ASAM brief triage screening.

Requirements: must be a registered or certified counselor with RADT/CADC/CATC or have a Masters in Social Work or Marriage & Family Therapy.

Director of Clinical Programs

This position at HHCLA supports the administration of behavioral health and related services of the agency, providing administrative, supervisory and clinical support for various behavioral health programs and staff. This individual will provide clinical direction and guidance for programming and training and support for clinical staff to help ensure the integrity of services being provided. Ensures the quality and financial health of programming and supports fundraising-related tasks. The position will run the Integrated Treatment Program and other behavioral health programs including the internship program and support at our Beverly location, primarily, while absorbing and supporting other responsibilities in time.

Substance Use Specialist I

 

The Substance Use Specialist I is responsible for conducting screenings/assessments, educating clients/patients about their different treatment options and facilitating individual counseling and group sessions, provides case management services, develops treatment plans, coordinates care and works with inter-disciplinary team to provide continuity of care.

Psychiatrist - Part Time

 

The Psychiatrist conducts initial psychiatric evaluations with referred clients, provides ongoing follow-up for clients accessing psychiatry services at the agency, coordinates with assigned Case Management staff working with referred clients, and offers recommendations to the agency on best practices. This is a part-time position.
