The Access to Housing for Health (AHH) Pilot Project is a collaboration between Homeless Health Care Los Angeles, Los Angeles County Department of Health Services, Housing Authority City of Los Angeles, Housing Authority County of Los Angeles, and Del Richardson & Associates. The ultimate goal of the Pilot Project is to provide permanent affordable housing linked to case management and other appropriate services for homeless individuals who are either frequent users of the Los Angeles County Hospital system or whose discharge is delayed due to their homeless status and medical fragility. The AHH program will help to improve participant's access to permanent housing, medical care, mental health care, alcohol/substance use treatment and other supportive services.

Upon acceptance into the AHH program, Homeless Health Care Los Angeles will provide a voucher for the participant to reside in a hotel, or find another form of transitional housing arrangement, until they have been approved for a Section 8 voucher through the City or County Housing Authority. Del Richardson and Associates will assist the participant in securing appropriate housing in the part of the city/county that they choose to reside and facilitate placement. Homeless Health Care Los Angeles will assist the participant into their permanent living arrangement and continue to provide them with ongoing intensive and comprehensive case management services.
